Should I use only the major arcana?
That is the most widespread practice for this spread: three cards drawn from the 22 majors. They carry broad archetypes, readable even by a beginner.
Nothing stops you using all 78: the reading gains nuance but becomes markedly harder to hold together across only three cards.
What if the third card is negative?
It does not say what will happen, but where the situation is heading if nothing changes. That is exactly where the spread earns its keep: it shows the point where you can step in.
Reread it in the light of the second card: the lever is almost always in the present.
Can I draw a fourth card?
Yes, as a synthesis card, when the first three contradict each other. But only one, and only once: stacking cards until the spread says something else is the surest way to make it unreadable.
